The short version

Petrolatum is a highly occlusive moisturizer: it forms a film that can reduce water loss from the skin surface, and dermatology reviews describe its broad topical use and low allergenic potential (Kamrani et al., 2024). That cosmetic function is not a claim that petrolatum treats acne, eczema, psoriasis or another disease.

The safety answer is conditional on what grade and refinement you are talking about. The strongest evidence applies to appropriately refined white petrolatum used as intended. It should not be merged with evidence about crude oil, used cutting fluids, or untreated and mildly treated industrial mineral oils. IARC’s monograph documents a very different hazard history for those occupational materials (IARC, Mineral Oils, Untreated or Mildly Treated).

So “petroleum-derived” is not a complete risk assessment, and “safe” is not a blanket certification of every product with petrolatum on the label.

What the grade distinction means

The FDA’s OTC skin-protectant monograph lists petrolatum and white petrolatum among permitted active ingredients at 30–100%, subject to the monograph’s conditions and labeling (FDA OTC Monograph M016). That is regulatory recognition of an ingredient and a use category. It is not the same as FDA preapproval of every cosmetic product, every brand or every lot, and it does not establish that a product treats a disease (FDA OTC Monograph M016).

In the EU cosmetics context, a 2019 review describes refinement-history, purity and pharmacopoeial conditions for permitted mineral oils and waxes, while also warning that “highly refined” is a quality category rather than a guarantee about a named brand (Chuberre et al., 2019). The review has disclosed medical-writing funding and author relationships with Cosmetique Active International, so it is best read alongside regulator and independent sources rather than alone (Chuberre et al., 2019).

The occupational-mineral-oil scare, separated correctly

IARC’s evidence is the reason a careful answer should not say “all petroleum is harmless.” Its monograph reports sufficient human evidence for skin cancer in historical occupational exposures to untreated or mildly treated mineral oils, where refinement and contamination varied (IARC, Mineral Oils, Untreated or Mildly Treated). The component evidence includes historical case series and worker cohorts, but those settings involved industrial materials and exposures that cannot simply be relabeled as a cosmetic white-petrolatum product (IARC, Mineral Oils, Untreated or Mildly Treated).

The same IARC chapter reports that white oils and petrolatums made using the most severe acid or hydro-treatment showed no activity in the cited skin-tumor bioassays, while human evidence for highly treated oils remained insufficient for a cancer conclusion (IARC, Mineral Oils, Untreated or Mildly Treated). That is the non-obvious receipt: both findings can be true because the variable doing the work is refinement and composition, not the emotionally loaded word “petroleum.”

This still is not a mathematical zero-risk finding. Historical occupational studies do not model ordinary leave-on cosmetic use, and an animal bioassay is not a certificate for every retail lot (IARC, Mineral Oils, Untreated or Mildly Treated).

What the broader safety reviews say

A review by authors from Germany’s Federal Institute for Risk Assessment distinguishes mineral-oil saturated hydrocarbons from aromatic hydrocarbons and emphasizes that identity and refinement matter (Pirow et al., 2019). For highly refined mineral oils and waxes used in consumer products, the review found no indication of carcinogenic effects in the animal evidence it assessed, while noting incomplete data for less refined or poorly characterized materials and different considerations for oral exposure (Pirow et al., 2019).

The German Federal Institute for Risk Assessment likewise says processing reduces or removes potentially carcinogenic hydrocarbons and considers health risks from consumer dermal absorption of mineral oils in cosmetics unlikely, while making clear that composition and refining are the reason for that conditional conclusion (BfR, Questions and Answers on Mineral Oil in Cosmetic Products). That regulator summary cannot verify a particular manufacturer’s refining history or lot-specific contamination without documentation (BfR, Questions and Answers on Mineral Oil in Cosmetic Products).

A 2024 dermatology review describes petrolatum as an anhydrous occlusive moisturizer and reports exceptionally rare contact allergy, while also noting that grade, purity, context and individual response matter (Kamrani et al., 2024). It does not prove zero allergy, zero comedogenicity for everyone, or zero cancer risk for every petroleum-derived material (Kamrani et al., 2024).

How to read a product claim

The FDA monograph’s listing of petrolatum does not mean the agency has approved every jar carrying a familiar brand name (FDA OTC Monograph M016). Do not infer a specific brand’s grade, purity or lot history from a generic internet reassurance; verify the manufacturer’s stated specification if that distinction matters.

Exposure matters too. Applying a refined ointment to intact skin, using a skin-protectant product on a minor scrape, and repeatedly ingesting a lip-care product are not identical scenarios. The mineral-oil review specifically warns that dermal and oral routes differ and that lip-care exposure can involve ingestion (Pirow et al., 2019).

The calm conclusion is therefore narrow: an appropriately refined white petrolatum product has a better-supported safety case for intended topical use, but no responsible source turns that into a guarantee about every petroleum-derived material, every formula or every person.
