The short version

Centella asiatica, also called gotu kola, does not become a standardized modern cica serum simply because a product uses the same plant name. A formula study still has to match the material being sold.

The four names shoppers see most often—asiaticoside, madecassoside, asiatic acid and madecassic acid—are related but distinct compounds. A label that names only “Centella extract” does not tell a shopper that it matches the compound and dose in a study, and evidence for an isolated compound cannot simply be assigned to every leaf extract (Kongkaew et al., 2020).

What the human formula studies found

One study applied oil-in-water cream and hydrogel formulas containing 2.5% or 5% Centella extract to the forearms of 25 volunteers twice daily for four weeks. The 5% formulations showed the strongest increase in measured surface hydration and decrease in transepidermal water loss among the tested concentrations (Ratz-Łyko et al., 2016). That is a narrow cosmetic signal from a named extract and vehicle, not a universal cica promise.

A separate intra-subject study used a complete fluid containing 1% hyaluronic acid, 5% glycerin and Centella stem-cell extract. A single application increased forearm hydration for up to 24 hours and reduced measured water loss against an untreated site, but the Centella concentration was not reported and the study could not separate it from the humectants (Milani & Sparavigna, 2017).

The label audit that matters

The most useful receipt is the mismatch between three exposures: a 5% whole extract study, a hyaluronic-acid-and-glycerin fluid with an undisclosed amount of stem-cell extract, and a separate photoaged-skin study of 0.1% madecassoside plus 5% vitamin C. They are not one interchangeable “Centella study” (Ratz-Łyko et al., 2016; Milani & Sparavigna, 2017; Haftek study summary).

That difference is important when a product highlights madecassoside but cites research on a whole extract, or highlights “stem cells” while the human result could largely reflect 1% hyaluronic acid and 5% glycerin. The study materials, concentrations and vehicles are the evidence—not the shared marketing nickname (Kongkaew et al., 2020).

How strong is the wider evidence?

A systematic review and network meta-analysis included five double-blind trials with 172 Asian women. The studies reported favorable wrinkle or hydration signals, but preparations varied, reporting was weak, and the authors said that lack of standardization prevented broad generalization (Kongkaew et al., 2020).

There is also a useful comparison study: 30 Indonesian batik workers applied a Centella cream or a ceramide cream to their hands and arms for four weeks. Both improved selected hydration and acidity measurements, with no significant efficacy difference between them; the Centella concentration was not reported (Anggraeni et al., 2021). That supports “a formula may feel better hydrated,” not “cica beats every moisturizer.”

The reasonable cosmetic expectation

If a finished product suits the skin, a modest expectation is better hydration, comfort or a smoother-looking surface. It is not reasonable to infer that every cica product contains all four centelloids, that a compound study proves a whole-extract result, or that a visible comfort claim is a medical one (Ratz-Łyko et al., 2016).

The reviewed records did not identify an adequately powered independent trial matching a named mainstream cica product. Extract standardization, vehicle and individual tolerance remain product-specific questions.
